   About the ServiceAlbany Community Care Centre is funded by Home and Community Care (HACC) which is a State and Community initiative to support aged, frail and people with a disability to remain living in the community. The service has been operating in Albany since 1986 and has grown from providing day care to currently including a range of community based services. The service is supported by paid staff and volunteers. We are continually working towards increasing our services to meet the changing needs of the community. All prospective clients are assessed by a staff member to determine eligibility for services under the HACC guidelines and policy. Provision of service will be directed by client choice and need. Information regarding service options and fees will be discussed at initial assessment. Changes to client needs can be reviewed at any time. All information will remain confidential. |
